A REPEAT PRESCRIPTION?

order a repeat prescription by using your repeat prescription slip or by telephoning
638 0117. If using the slip, tick the items required and post it to us or hand
it into reception.
If you enclose an SAE we will post it back to you. If using the telephone line,
please speak clearly and leave your name, address, date of birth and details of
